About Us

Daily Grace Adult Day Health Services is a nurturing Adult Day Care facility that offers transitional care, caregiver respite care, and short-term rehabilitation for disabled adults and seniors. We care for persons age 18 and older with cognitive and/or physical disabilities. Physical and Occupational Therapy are available as well. We are able to manage chronic diseases such as Cardiac disease, Diabetes, Renal Disease, Dementia, or Respiratory disease. Our services require a referral from the persons Medical Doctor. Our Registered Nurse establishes the Care Plan including the client's and/or caregiver's input for care that's needed. Our service is a great alternative to Nursing Home Placement. And, a great addition to Homecare or Home Health Care. If you have a loved one that would like to return home from a nursing home but needs day-time care and supervision consider our facility to be a part of their Care Team. If a Caregiver needs to work but their loved one is not safe to live at home alone, our facility is a nurturing environment for them to come to. Clients can stay for a Half Day or Full Day.
